We commend the SPRINT (Systolic Blood Pressure Intervention Trial) team1 for including a mechanistic evaluation in the SPRINT trial, reported by Soliman et al.2 In addition to demonstrating clinical effectiveness, understanding how interventions work allows us to determine which are the primary disease processes and which are downstream effects.
