We read with great interest the recent publication by McEvoy et al1 on the significance of coronary artery calcium (CAC) to guide a personalized risk-based approach to the initiation and intensification of antihypertensive therapy. The authors should be commended for the quality of their data. Still, some limitations should be mentioned in addition to those acknowledged by the authors.
