Abstract 15876: Symptoms Consistent With Vagus Nerve Injury Post Radiofrequency Catheter Ablation for Atrial Fibrillation
Introduction: The vagus nerve injury during catheter ablation for atrial fibrillation (AF) can significantly impact quality of life and result in symptoms of dysphagia, gastroesophageal reflex disease, nausea, vomiting, bloating, early satiety, constipation, diarrhea, or decrease in gastric motility. Despite a relatively high incidence of these symptoms post ablation, rather little is known about the time course of symptom persistence or patient characteristics that predispose to injury. This study was designed to define risk factors of vagal nerve injury, symptom prevalence and temporal resolution.
Methods: A total of 100 patients undergoing radiofrequency catheter ablation (RFCA) were enrolled and consented to participate in the study. Patients completed a 22 item questionnaire that included questions specific to vagus nerve injury symptomatology during their baseline visit and then again at 1 and 3 month post-RFCA.
Results: The average age of the population was 63±10.6 years and 68% were male. Baseline characteristics are shown in the table. A total of 81 patients have completed their 1-month follow-up survey, with all patients having completed their trial participation by the AHA scientific sessions. Frequency of baseline and 1 month post-RFCA symptoms were: trouble swallowing (2.2% vs. 10.3%), bloating (5.5% vs. 14.6%), nausea (3.3% vs. 14.0%), constipation (3.4% vs. 6.3%), and feeling full (5.5% vs. 19.6%), respectively.
Conclusion: Gastrointestinal disease symptoms are prevalent in patients than undergo AF ablation. The majority of vagus nerve injury symptoms increases significantly after ablation and can be debilitating. This survey administered to a large cohort of patients will provide a more comprehensive understanding of the risk factor for vagus nerve injury and provide practitioners insight into temporal resolution of symptoms.
