Abstract 15477: Predictors Associated With 3-Year Adverse Clinical Outcomes Following Successful Left Main Intervention With Drug-Eluting Stents
Background: There is limited evidence regarding predictors associated with long-term clinical outcomes in patients (pts) with left main disease successfully treated with drug-eluting stents (DESs).
Methods: A total 221 pts underwent successful PCI for left main disease with DESs from Apr 2004 to Apr 2012 were enrolled. All major adverse cardiac events (ALL-MACEs) were defined as composite of total death, any myocardial infarction (MI), target vessel revascularization (TVR) and non-TVR. Target lesion revascularization MACEs (TLR-MACEs) were defined as composites of cardiac death, Q-wave MI, and TLR. The association of ALL-MACEs, TLR-MACE, TLR, and cardiac death with clinical, biochemical and procedural variables including gender, age, hypertension, diabetes, hyperlipidemia, acute MI, chronic kidney disease, dialysis, cerebrovascular accident, peripheral arterial disease, current smoker, current alcoholics, prior MI, prior PCI, prior CABG, intravascular ultrasound (IVUS) usage, lesion calcification, statins, and the use of triple antiplatelet agents. Multivariate logistic regression was used to identify predictors affecting 3-year cardiac death, any MI, TLR, ALL-MACEs and TLR-MACEs.
Results: The incidence of cardiac death was 9.5% (21/221) any MI 5.9% (13/221), TLR 10.1% (26/221), stent thrombosis 1.9% (5/221), ALL-MACEs 33% (73 of 221), and TLR- MACEs 21.7 (48/221) during 3-year follow-up period. The presentation with AMI on admission was adverse predictor and IVUS usage, statin and triple antiplatelet medications were protective predictors for adverse clinical outcomes following successful left main PCI up to 3 years (Table).
Conclusions: In our study, presentation with AMI was an independent predictor, whereas the IVUS usage, statin and triple antiplatelets were protective predictors of adverse clinical outcomes following successful left main PCI with DESs.
