Abstract 14909: Difference of Five-year Clinical Outcomes of Multi-overlapped Stents for Treatment of Long Lesions Between First and Second Generation Drug Eluting Stents
Background: Previous studies indicated that long coronary lesions are one of the key predictors of drug-eluting stent (DES) failure, but long-term clinical outcomes of multi-overlapped long DESs are not well investigated. Thereby, we compared 5-year major adverse cardiac events (MACE) of multi-overlapped stents for long coronary artery disease between first and second generation DESs in the present study.
Methods: We enrolled the consecutive patients with multi-overlapped DESs (≥3 stents) for long coronary artery disease from July 2009 to May 2011. Study endpoints included MACE defined as the composite of all-cause death, in-stent restenosis of target lesion, as well as heart failure required hospitalization for five years. We compared age, gender, coronary risk factors including dyslipidemia, hypertension, diabetes mellitus, smoking and chronic kidney disease and the stent length and the stent size between the first-generation DES (FG) and second generation DES (SG) groups. In addition, we compared MACE free survival between the two groups using Kaplan-Meier method.
Results: FG group comprised 16 patients and SG group comprised 11 patients. The incidence of hypertension was significantly higher in SG group than FG group (p=0.014) while there were no significant differences of the other coronary risk factors between the two groups. In addition, the stent length and stent size were similar between FG group and SG group (81.1±19.0 vs. 77.2±19.0 mm and 2.91±0.17 vs. 2.84±0.19mm, respectively). As shown in a figure, MACE free survival for 5 years, in SG group was significantly higher than FG group (81.3% vs 36.4%, P=0.049).
Conclusions: In the patients with multi-overlapped DESs (≥3 stents) for long coronary artery disease, second generation DES was associated with lower rate of 5-year MACE as compared to first generation DES.
