Abstract 19974: Semi-automated Quantification of Left Atrial Size Using 3 Beat Averaging Real Time 3D Echocardiography in Patients with Atrial Fibrillation; A Validation Study
Introduction: Left atrial (LA) volume has an important prognostic value in patients with atrial fibrillation (AF). However, current recommendation for that is not easily assessable as multi-beat LA volume is time-consuming process.
Hypothesis: We assessed the feasibility and accuracy of an automated 3-beat averaging real time three-dimensional echocardiography (RT3DE) in patients with AF.
Methods: 31 patients with AF (62.8±11.7 years, 67.7% male) were prospectively recruited to have a 2D echocardiography (2DE) and 3-beat averaging RT3DE (SC 2000, Acuson, USA). Maximal LA volume was measured by conventional area-length method on 2D TTE and semi-automated LA volume measurement using dedicated software (eSie LVATM volume analysis) (Siemens Medical Solution, Mountainview, USA) and compared with computed tomography (CT) scan by reference. Pearson’s correlation and Bland-Altman analysis were performed.
Results: LA volume using RT3DE was feasible for all patients. Measured LA volume was 65.4±32.1 by 2DE, 92.5±25.2 by RT3DE, and 117.6±36.0mL by CT, respectively. In overall patients, LA volume from 2DE (r=0.12, p=0.52) or RT3DE (r=0.23, p=0.13) and CT showed poor correlation. However, among good image quality group (n=16), RT3DE showed significantly good correlation with CT (r=0.78, p<0.001).
Conclusions: Rapid and accurate automated quantification of LA volume is feasible using 3-beat averaging RT3DE in patients with AF. The good image quality was essential to maximize its value. This technique promises to improve integration of RT3DE into routine clinical practice.
