Abstract 12154: 10-Year Development in Cardiac Mortality in the Young - A Nationwide Cohort Study in Denmark
Background: Reliable and timely information on incidence rates and causes of death in the young is crucial for developing strategies to lower mortality. We examined temporal changes in nationwide mortality rates as well as cause of death in individuals aged 1-35 years.
Methods: All deaths in Denmark between 2000 and 2009 in persons aged 1-35 were identified using the nationwide Danish Civil Registration Registry. Death certificates were retrieved electronically and reviewed by two physicians. Autopsy reports were read and cause of death was established. Incidence rates were calculated using the background age group population as the denominator. Changes in incidence rates were analysed by Joinpoint regression models.
Results: During the study period, there were 9,056 deaths and death certificates were available in 95% (n=8,638) of cases. The median age was 26 years; 69% were men. All-cause mortality decreased from 43.5 to 32.8 per 100.000 person-years in 2000 and 2009, respectively. This corresponds to an annual percentage change (APC) during the study period of -3.2 (-4.0 to -2.5, p<0.01) (Figure 1). Cardiac diseases were the most common cause of natural death (n=1,114, 27%) followed by pulmonary diseases (n=764, 19%). The incidence rate of cardiac death was 4.7 per 100,000 persons-years with a decrease over time: APC -3.7 (-8.0 to 0.8, p=0.09). In 635 cases (57%), the cardiac death was sudden and unexpected (sudden cardiac death, SCD) corresponding to an incidence rate of 2.7 per 100,000 person-years. The incidence rate of SCD also decreased significantly: APC -3.6 (-6.9 to -0.3, p<0.05).
Conclusion: All-cause mortality decreased from 2000 to 2009 in the young in Denmark. Cardiac diseases were the predominant cause of death and the incidence rate decreased significantly during the study period. The current findings may indicate that the management of young patients with cardiac diseases have improved.
