Abstract 16497: Prediction of Left Atrial Appendage Thrombus by Spaced in Comparison to Chads2And Cha2Ds2-vasc. Validation Study

Abstract
BACKGROUND.SPACED (Stroke, Permanent AF, Atrial Fibrillation duration, CHF, presence of sEc, and Diabetes mellitus) is a novel risk assessment tool to determine the thromboembolic propensity of non-valvular atrial fibrillation (NVAF). We are now provide prospective validation of this tool compared to CHADS2 and CHA2DS2-VASc.
METHODS. SPACED, CHADS2and CHA2DS2-VASc were compared by logistic regression analysis for the prediction of left atrial appendage thrombus (LAAT) in consecutive, warfarin naïve NVAF patients undergoing TEE (5/7/2003 - 10/21/2009).
RESULTS. 429 patients (mean age 64+15; 30% female) participated (Table). LAAT discrimination (Figure) was significantly greater for SPACED compared to CHA2DS2-VASc or CHADS2.
CONCLUSION. By including rhythm duration, AF type and a measure of blood stasis, SPACED both embraces the pathophysiology and provides an accurate improved risk discrimination for thromboembolic propensity in NVAF.
