In the article by Creager et al, “2012 ACCF/AHA/ACR/SCAI/SIR/STS/SVM/SVN/SVS Key Data Elements and Definitions for Peripheral Atherosclerotic Vascular Disease: A Report of the American College of Cardiology Foundation/American Heart Association Task Force on Clinical Data Standards (Writing Committee to Develop Clinical Data Standards for Peripheral Atherosclerotic Vascular Disease),” which published ahead of print on December 5, 2011, and appeared in the January 17, 2012, issue of the journal (*Circulation*. 2012;125:[395–467](http://circ.ahajournals.org/lookup/volpage/125/395)), several corrections were needed: 

1.  On page 395, in the title, the Society for Vascular Surgery (SVS) was added. The title read, “2012 ACCF/AHA/ACR/SCAI/SIR/STS/SVM/SVN Key Data Elements ….” It has been changed to read, “2012 ACCF/AHA/ACR/SCAI/SIR/STS/SVM/SVN/SVS Key Data Elements ….”

2.  On page 395, in the footnotes, second paragraph, the Society for Vascular Surgery was added to the organizations approving the Data Standards document. The sentence read, “This document was approved by … Society for Vascular Nursing in October 2011; and the American College of Radiology and Society for Cardiovascular Angiography and Interventions in November 2011.” It has been changed to read, “This document was approved by … Society for Vascular Nursing in October 2011; the American College of Radiology and Society for Cardiovascular Angiography and Interventions in November 2011; and the Society for Vascular Surgery in December 2011.”

3.  On page 395, in the footnotes, third paragraph, in the citation title, the Society for Vascular Surgery (SVS) was added. The citation information read, “The American Heart Association requests that this document be cited as follows: Creager MA, … 2012 ACCF/AHA/ACR/SCAI/SIR/STS/SVM/SVN key data elements ….” It has been changed to read, “The American Heart Association requests that this document be cited as follows: Creager MA … 2012 ACCF/AHA/ACR/SCAI/SIR/STS/SVM/SVN/SVS key data elements ….”

4.  On page 398, in the first column, first paragraph under the heading, “2.2. Relationships With Industry and Other Entities,” line 18, the sentence “In addition, to ensure complete transparency, writing committee members' comprehensive disclosure information—including relationships not pertinent to this document—is available online as a supplement to this document.” was deleted.

5.  On page 399, in the first column, first paragraph, the first full sentence, the Society for Vascular Surgery was added to the organizations approving the Data Standards document. The sentence read, “The document was then approved by … Vascular Disease Foundation in October 2011; and the American College of Radiology and Society for Cardiovascular Angiography and Interventions in November 2011.” It has been changed to read, “The document was then approved by … Vascular Disease Foundation in October 2011; the American College of Radiology and Society for Cardiovascular Angiography and Interventions in November 2011; and the Society for Vascular Surgery in December 2011.”

6.  On page 464, in the title for Appendix 1, the Society for Vascular Surgery (SVS) was added. The title read, “Appendix 1. Author Relationships With Industry and Other Entities—2012 ACCF/AHA/ACR/SCAI/SIR/STS/SVM/SVN Key Data Elements ….” It has been changed to read, “Appendix 1. Author Relationships With Industry and Other Entities—2012 ACCF/AHA/ACR/SCAI/SIR/STS/SVM/SVN/SVS Key Data Elements ….”

7.  On page 466, in the title to Appendix 2, the Society for Vascular Surgery (SVS) was added. The title read, “Appendix 2. Peer Reviewer Relationships With Industry and Other Entities—2012 ACCF/AHA/ACR/SCAI/SIR/STS/SVM/SVN Key Data Elements ….” It has been changed to read, “Appendix 2. Peer Reviewer Relationships With Industry and Other Entities—2012 ACCF/AHA/ACR/SCAI/SIR/STS/SVM/SVN/SVS Key Data Elements ….”

8.  On page 467, in the Appendix 2 footnote, last paragraph, the definition of “SVS” was corrected. The paragraph read, “AACVPR indicates … SVS, Society of Vascular Surgeons ….” It has been changed to read, “AACVPR indicates … SVS, Society for Vascular Surgery ….”

9.  The online-only data supplement, titled “Comprehensive Relationships Table,” was removed from the *Circulation* Web site because it duplicated information in Appendix 1. This joint document was initiated before the comprehensive disclosure information policy was enacted, and comprehensive disclosures were not collected. The online-only data supplement read:

It is the policy of the American College of Cardiology Foundation and the American Heart Association to make available online complete disclosure of relationships with industry and other entities of the writing committee, including relationships not pertinent to a document. Because the “2012 ACCF/AHA/ACR/SCAI/SIR/STS/SVM/SVN Key Data Elements and Definitions for Peripheral Atherosclerotic Vascular Disease” document was initiated before this policy was enacted, comprehensive disclosures were not collected. See Appendix 1 of the article for the relationships of writing committee members with industry and other entities that were reported by authors to be relevant to the document.
