Abstract 16612: The Efficiency of 1.5T Non-contrast Whole Heart MR Coronary Angiography by 32-channel Cardiac Coil for Non-invasive Screening Method of Coronary Artery Disease
Background: 1.5T whole heart MR coronary angiography (WH-MRCA) is practicable without contrast and radiation-exposure. WH-MRCA can combine all of safety, diagnostic performance and economic efficiency and has a potential as ultimate non-invasive screening method for coronary artery disease (CAD). However, the scan time is longer than CT coronary angiography (CTCA) and the dissemination is not enough. Recently developed 32-channel cardiac coil can reduce scan time to half. We prospectively evaluated an availability of non-contrast WH-MRCA using this coil for screening of CAD.
Method: We performed WH-MRCA in 156 new patients without history of CAD (79 males, 77 females, 63.0±14.3 years old). Scan time was recorded and two independent investigators scored image (4:fine, 3:good, 2:acceptable, 1:poor). Diameters of major vessels were quantitatively measured by workstation. We performed CTCA or coronary angiography (CAG) in the cases with >75% dominant stenoses and compared with WH-MRCA. In the cases without stenoses, we followed up the occurrence of cardiovascular events. For complete non-invasive scan, premedication with beta-blocker to control heart was not administered.
Result: Heart rates were 75.1±13.7 bpm. Scan time was 6.2±2.9 minutes. Image quality score of RCA, LMT, LAD and LCx was 3.2±0.9, 3.5±0.6, 3.1±1.0 and 2.8±1.0. In the 14 patients, >75% dominant stenoses were found. The sensitivity, specificity, positive and negative predictive values were 83.3, 96.8, 78.9 and 97.6%. Follow-up duration of the cases without stenosis was 247.3±95.8 days and no cardiovascular event occurred except only one case of complete atrioventricular block. Event-free rate of the cases without stenosis was 99.3%.
Conclusion: WH-MRCA by 32-channnel cardiac coil could considerably reduce scan time with high negative predictive value. In conclusion, 1.5T non-contrast WH-MRCA provides possibility of complete non-invasive screening for cardiovascular disease.
