Abstract 3885: Relationship Between Catestatin and Cardiac Function
[Background] Catestatin is hydrolyzed from chromogranin A, inhibiting the release of catecholamine. It was reported that Catestatin can inhibit adrenerigic activity thus reduce cardiac output. However, the change of Catestatin level in heart failure and its association with cardiac structrual and functional alteration were still unkonwn.
[Methods] Establish rat myocardial infarction models with coronary artery ligation and rat pressure loading models with abdominal aortic ligation. Evaluate cardiac function by hemodynamics and echocardiography.75 cases admitted in Dept. of cardiology were recruited. They were divided into three groups according to the ACC classification. Detect the plasma Catestatin, BNP levels by ELISA method and use SPSS 13.0 to analyze.
[Results] Part One Catestatin levels of the control group (CR), early HF group (EHF) and late HF group (LHF) in rat myocardial infarction models were as follows: (6.41±4.37) ng/mL, (2.63±0.87) ng/mL, (2.37±1.33) ng/mL. Catestatin levels of EHF had declined significantly compared with CR, but there were no significant differences between EHF and LHF. Catestatin levels of EHF (2.81±0.95 ng/mL) and LHF (2.67±0.85 ng/mL) were lower than CR in rat pressure loading models, but there were no significant differences. BNP levels were associated with the severity of heart function and BNP of ischemic LHF remarkably elevated than CR. Part Two Catestatin levels in stage A (n=27), B (n=19) and C (n=29)groups were as follows: (21.28±7.15) ng/mL, (13.66±4.28) ng/mL and (13.41±5.01) ng/mL. They had significantly reduced in stage B and C groups compared with stage A group (P<0.001), but there were no differences between stage B and C group. Catestatin of I-level B (NYHA I, ACC B) group had declined significantly (P<0.001) compared with that of I-level A (NYHA I, ACC A) group. ROC curve showed when cutoff value was 17.022 ng/mL, the sensitivity was 93.7% and the negative predictive value was 94.4%. BNP of stage C group had significantly elevated than stage A group.
[Conclusions] Catestatin is sensitive in reflecting cardiac structrual changes. It could be used in screening early stage (A–B) heart failure. BNP combined with Catestatin will classify cardiac function better.
