Abstract P23: EuroSCORE is predictive of Postoperative Atrial Fibrillation After Coronary Artery Bypass Graft Surgery
Background: Postoperative atrial fibrillation (AF) is the most common arrhythmia after cardiac surgery and is a significant factor in post-operative morbidity. The European System for Cardiac Operative Risk Evaluation (EuroSCORE), a method for calculating predicted operative mortality in patients undergoing cardiac surgery, was reported to be a predictor of postoperative AF in elderly patients undergoing coronary bypass surgery in a single study of 426 patients.
Aim: to investigate whether EuroSCORE is predictive of postoperative AF after isolated coronary artery bypass surgery in a larger population
Methods: We reviewed data from our prospective cardiac surgery database of all patients undergoing cardiac surgery from January 2003 through December 2006 at a university medical center. A total of 2,791 patients were studied. A stepwise logistic regression model was used to identify independent predictors of postoperative AF.
Results: The mean age was 68±9.1 [mean±SD] yrs, and the mean EuroSCORE was 6.9±3.2. The overall incidence of postoperative AF was 32.2 %. Chi-square analysis revealed a significant association between post-operative AF and the EuroSCORE (p<0.0001). On univariate analysis, EuroSCORE was predictive of post-operative AF (p<0.0001; OR 1.099; 95% CI 1.07 to 1.12). The stepwise logistic regression model, used to identify independent predictors of postoperative AF, included the following variables: age, diabetes mellitus, EuroSCORE, gender, ejection fraction, hypertension, and preoperative creatinine. Only age (OR 1.04, 95% CI 1.03 to 1.06, p<0.0001) and EuroScore (OR 1.03, 95% CI 1.001 to 1.06, p=0.045) were independent predictors of postoperative AF.
Conclusion: EuroSCORE is predictive of postoperative AF after isolated CABG. Thus, utilization of the EuroSCORE system may help to identify those at risk for postoperative AF and guide prophylactic measures to reduce the incidence of postoperative AF.
