Abstract 2682: Differences in Atherosclerotic Profiles Between Patients with Abdominal and Thoracic Aortic Aneurysms.
Background Atherosclerosis is involved in the pathogenesis of aortic aneurysms. However, differences in the types of atherosclerosis between abdominal (AAA) and thoracic aortic aneurysms (TAA) have not been studied. The purpose of this study was to clarify the differences in the atherosclerotic profiles of patients with AAA and TAA.
Methods We reviewed 343 consecutive patients (210 AAAs and 312 TAAs who were admitted to our hospital for elective repair of aortic aneurysms between July 2001 and December 2004. Clinical records of these patients were retrospectively reviewed. Clinical variables were compared between patients with AAA and TAA by using a univariate analysis, and variables achieving statistical significance were subsequently assessed in a multivariate analysis.
Results The incidences of coronary artery disease (55 vs. 25 %, p<0.001), 3-vessel disease (28 vs. 3 %, p<0.001), male patient (86 vs. 74%, p<0.01), smoker (88 vs. 76 %, p<0.01), chronic obstructive pulmonary disease (30 vs. 15%, p<0.01), and diabetes (39 vs. 23 %, p<0.01) were significantly higher in AAA than in TAA. In contrast, the incidences of hypertension (91 vs. 81 %, p<0.05), saccular type aneurysm (61 vs. 7 %, p<0.001), and body mass index (BMI) were significantly higher in TAA than in AAA. Multivariate stepwise logistic analysis found that coronary artery disease (p<0.0001; OR 3.61; 95%CI 0.37– 0.93), chronic obstructive pulmonary disease (p<0.05; OR 2.08; 95%CI 0.06 – 0.69), and diabetes (p<0.05; OR 1.87; 95%CI 0.03– 0.06) were significantly associated with AAA; and that BMI (p<0.01; OR 9.09; 95%CI 0.03– 0.19), hypertension (p<0.01; OR 3.13; 95%CI 0.19 – 0.96), and history of cerebral infarction (p<0.05; OR 2.86; 95%CI 0.11– 0.94) were associated with TAA.
Conclusions Atherosclerotic profiles are different between patients with AAA and TAA. These findings suggest that mechanisms of developing of aortic aneurysms may be different between AAA and TAA.
