Hazard ratios were adjusted for age, or multifactorially for age, total cholesterol, low density lipoprotein cholesterol, high density lipoprotein cholesterol, triglycerides, apolipoprotein B, body mass index, hypertension, diabetes mellitus, smoking, lipid lowering therapy and for women also menopause and hormone replacement therapy. There was no adjustment for regression dilution bias. P values are test for trend of hazard ratios.
